Documents, bills & coverage

Carry the right papers, understand how billing works, and confirm insurance steps early—especially before planned admission or surgery.

Documents & medical records

Identity & basics

  • Government photo ID (Aadhaar, voter ID, passport, or driving licence) for the patient.
  • Recent passport-size photographs if admission or pre-auth forms require them.
  • Insurance / TPA card (if applicable) and employer letter where your policy asks for it.

Clinical papers

  • Old prescriptions, discharge summaries, and operation notes from other hospitals.
  • Prior audiometry, imaging (CT/MRI reports), or lab reports related to your ENT condition.
  • List of current medicines, allergies, and chronic illnesses (diabetes, hypertension, etc.).

Note: Keep copies of all bills and summaries for insurance claims and medico-legal needs. The hospital issues prescriptions and discharge documents as per standard practice; ask reception if you need duplicate copies.

Billing & payments

Charges & estimates

  • Consultation fees, procedure charges, room rent, OT charges, implants, and medicines are billed as per hospital tariff and your treatment plan.
  • Where possible, ask for an estimate before elective admission or surgery; final bills may vary with actual consumables and length of stay.
  • Payments may be accepted by cash and modes supported at the desk—confirm current options with billing.

Transparency

  • Request an itemised bill and clarify any line item you do not understand before discharge.
  • Keep all payment receipts and the final bill copy for your records and insurance.
  • For queries on refunds or corrections, speak to the billing counter during working hours.

Insurance & TPA (cashless)

Cashless & reimbursement

  • Cashless treatment depends on your insurer, policy, and our hospital’s tie-up status—please confirm with the desk before admission or surgery.
  • Pre-authorisation is usually required for planned procedures; carry your policy card, ID, and any forms your TPA or employer provides.
  • For reimbursement claims, collect signed bills, discharge summary, and investigation reports as listed by your insurer.

Important

Insurance rules change by company and product. The hospital assists with documentation and billing formats, but approval and claim settlement are between you and your insurer / TPA. Ask billing for the latest list of empaneled networks and any deposit requirements for cashless cases.
